How we use the information we collect
Your information will be used by the Forever Smile Clinic in order to carry out the patient scheduling process, receiving and identifying patients when they come to the appointment, establishing medical recommendations by the specialist.

Creating and / or submitting reports in accordance with local laws and / or regulations, as appropriate;
Improving the application and / or customer support process of the Forever Smile Clinic.
Compliance with applicable laws, regulations, lawsuits or government requirements.
In some cases, we will also use your information to protect the rights and property of the Forever Smile Clinic, the rights, employees or other interested parties, as required by law.

How long will we keep our personal data?

Unless you object to the processing of personal data by us and / or request the deletion of your personal data, the following retention periods, in accordance with our data retention policy, apply to the following categories of personal data:

  • We do not store your personal data for longer than is necessary for the purpose for which we retained it or to comply with a statutory law or requirement.

  • Patients' data will be kept for at least 10 years, in order to be able to fiscally prove the payment operation of the services provided.

Your legal rights

As a data subject, you have specific legal rights regarding the personal data we collect from you. The Forever Smile Clinic will respect your individual rights and take care of your interests accordingly.

  • Right of withdrawal of consent: You may withdraw your consent to the processing of personal data at any time.

  • Right to rectification: You can obtain from us the rectification of personal data concerning you. We make reasonable efforts to keep personal data - which are used continuously and in our possession or control - accurate, complete, current and relevant, based on the latest information available to us.

  • Right to restriction: You can obtain from us the restriction on the processing of personal data, if:

  • challenge the correctness of personal data for the period in which we must verify the accuracy,

  • the processing is illegal and you request the restriction of the processing rather than the deletion of personal data,

  • we no longer need your personal data, but you request it to establish, exercise or defend a right, or

  • you object to the processing while we verify that our legitimate reasons take precedence over yours.

  • Right of access: You can ask us for information about the personal data we hold about you, including information about what categories of personal data we have in our possession or control, what they are used for, where we have them from. collected, if they are not collected directly from you and to whom they were disclosed, if applicable. You may obtain a copy from us, free of charge, containing the personal data we hold about you. We reserve the right to charge a reasonable fee for each additional copy you may request.

Rather than receiving a copy of your personal data, you may request that we transfer your data directly to another operator specified by you.
